I can think of 10 (There are probably more) Different Palyers who have all scored for 5 different teams while in the top flight.
All players must have scored in the Premiership.....and you must name the teams they have scorde for.

Frank Sinclair, he scored for Chelsea whilst playing for Leicester, he scored for Arsenal whilst playing for Leicester, he scored for Middlesbrough whilst playing for Leicester, I think he might have actually scored for Leicester aswell at some stage. That leaves just one other team he must have scored for at some stage :D

He hasn't scored for West Ham yet ...
He is also famous for sharing the record with a certain Mr Ward for being relegated the most from the Prem a massive 4 times although he broke his run with the Hammers staying up on the last day of the season...
